My brother is 15.5 years old. He has had high fever (102–103°F) since morning, vomiting, dry cough, and feeling cold/shivering.
I gave 650 mg paracetamol and one dose of amoxicillin (no improvement). Also did cold sponging.
Now fever is again 103°F with vomiting and cough.
Please guide urgently — which medicine is safe to give now? We can't go to a hospital right away.

Just one dose of antibiotic will not help immediately
Better to take tab amoxy + clauvalanic acid 625mg one morning and one evening for 5days.
If vomiting is with cough, it shall improve with cough, if separately, give tab dom d.t., 10mg one every 8 hours for 4 days. Give electral powder.
If vomiting is much, injectable treatment is needed with I.V. fluids, in a hospital facility.
Give him plenty of fluids, let him take hot saline gargles.
For fever, you can do tepid sponging tab paracetamol 500mg every 6 to 8 hours.
Get cbp and malaria antigen test, share results.

Tab montair lc daily night time.
Adequate hydration.
Normal water with salt.for gargle..

Fever since morning ?? Just give paracetamol 3 times to 4 times daily and don’t initiate amoxicillin yourself without consulting doctors .. ensure adequate hydration give more liquids of any type juice / tender coconut though he is vomitting .. and few more questions does he have chills or shaking ?? Any body pain or muscle pain ? Any headache ? Any abdominal pain ? Any rashes starting on his body ? Any loose stools ?? Does he able to eat normal food ? Is he active ?? You said cough. How it is dry or with phlegm ? ?? Any breathing trouble or any pain in the chest anywhere ? Useually viral fever will settle in 1 or 2 days with paracetamol and hydration . You can take Levocitration and take plenty of liquids and rest , if persistent fever along with warning signs like rapid breathing or chest pain , etc consult nearby if it persists need blood tests too
