Hie.. yea ..it’s carious ..I would suggest to get it remove since I could see the caries is approaching your root also.. and there is no point in saving this tooth since it doesn’t help in chewing also ..
Next Steps
Kindly consult or visit dentist

As this is the 3rd molar , it should be removed. Because it doesn't aid in eating food, further its difficult to maintain it clean that's y the decay.  So it's better to get it removed
Next Steps
book an appointment for tooth removal
Health Tips
if its paining , start with antibiotics and pain killer, as within this medication duration, tooth can be removed and no need to take extra medication course

Hi. I assume your concern is regarding the upper third molar  (last tooth). And yes it's better to get rid of that tooth if it is giving you trouble.
Next Steps
Visit the dentist and fix appointment for tooth removal.
Health Tips
Not related to your question, but i sense some abnormality related to lower right canine. If possible please have a check up done with your dentist clinically to rule out any pathology related to that tooth. ( Don't panic - just a piece of advice as an observation from the X-ray provided here). Take care.
