Tsh is so high ,indicated that hypothyroidism and its causes weight gain. Also vitamin d level is low .lipid profile is manageable by lifestyle measures. Others like d3 vitamin you should take uprise 60 k weekly.and levothyroxine 50mcg for thyroid.
