Firstly there is no home remedy to treat cavities.Tooth area once lost is lost forever it would not regenerate but the good thing is it could restored by dental cements for proper aesthetics and functioning. Also for plaque you need to get professional cleaning done.

Plaque has no home remedy, you have got to go to the dentist fot scaling. But again, diagnosis on your own is never helpful, sensitivity can be because of anything else. And cavities also need to be diagnosed properly for how deep they are so that they can be treated fine.
Next Steps
visit dentist

Hi! For plaque- no home remedy. Do a professional scaling done once in 6 months and after that procedure home care of twice brushing, flossing and rinsing after each meal - every day will help. Tooth cavities definitely have to go for treatment by removal of the decay part and filling it back with cement!
Next Steps
Visit a dentist for the further procedure.
Health Tips
Treat the teeth at the earliest and don't delay before the condition worsens. Take care.
