I have gone through your description.
Fall down of deciduous tooth and eruption of permanent tooth is a natural process. Every child has going through this phase.
Falling of deciduous tooth is starting from at the age of 7-9years, upper front tooth.
Sometimes, this tooth fall early before tha age. It's a quite normal thing you shouldn't worry about This.
This moving tooth is falldown in few days. That doesn't mean permanent tooth is start to erupt.
Permanent tooth is start to erupt between 7to9 yrs of age. Don't get tens if permanent tooth is not erupt after falling that tooth.

See there is normal variation of extraction and eruption of teeth by 2-4 years.so in case in the age of 4 years if decidous tooth is getting exfoliated it is not abnormal.however for post extraction precautions, you can consult me

You can evaluate the condition by clinical examination & radiographic investigation to check whether underlying front teeth are erupting or not by visiting the dentist / pedodontist as age of eruption of upper permanent front teeth is 7 - 8 yrs for central incisors & 8-9 yrs for lateral incisors.
