The patient's sequential TSH levels demonstrate a reassuring, progressive decline (14.64 → 12.4 → 9.93). Current biochemical data does not meet the threshold to initiate thyroid hormone replacement therapy.
Assessment & Plan:
Current Status: Mild, residual TSH elevation at 25 days of life.
Action Required: Repeat serum TSH and Free T4 (FT4) in approximately 7 days.
Prognosis
If TSH continues to normalize and FT4 remains stable, clinical observation will suffice.
If TSH remains persistently elevated or FT4 falls below standard reference ranges, further diagnostic evaluation and therapeutic intervention will be initiated.

Repeat Tsh and free T4 after 1 week .
As der is decrease in tsh levels just monitor values to initiate therapy .
If tsh decreasing with normal range f t4 . No need to start therapy

Reports are showing a gradually decreasing TSH (14.64 → 12.4 → 9.93), which is reassuring. At present, there is no definite indication to start thyroid medicine based on these reports alone. However, since TSH is still mildly elevated at 25 days of age, it should be followed up.
Please repeat TSH and Free T4 after about 1 week. If TSH continues to decrease and Free T4 remains normal, usually only follow-up is required. If TSH remains persistently elevated or Free T4 becomes low, further evaluation and treatment may be required.
So currently, no need to panic, but please don’t skip the repeat thyroid test.
