You seem to be having diffuse goitre according to ultrasound report. Probably a auto immune condition known as chronic lymphocytic thyroiditis also known as hashimotos thyroiditis. You need to get thyroid function tests and fine needle aspiration cytology of thyroid to confirm the diagnosis. Usually treatment is with thyroid hormone replacement medications
