The diagnosis of β-thalassemia is confirmed in a patient over 12 months of age by observing microcytic hypochromic anemia, ruling out iron deficiency, noting anisopoikilocytosis with nucleated red blood cells on a peripheral blood smear, and detecting decreased or absent hemoglobin A (HbA) with elevated hemoglobin A2 (HbA2) and often elevated hemoglobin F (HbF) upon hemoglobin analysis.
