A person who is actually "mad" in layman terms (which usually means the person may be having Schizophrenia) will seldom have the realisation that he is "talking to self" or "smiling for no reason". The person may be hearing voices (having auditory hallucinations) to which he responds, which others see as "talking to self" etc. In any case, it wouldn't hurt to visit a Psychiatrist and have your concerns addressed. A thorough evaluation will clear the picture up.

If you are talking to yourself or smiling in response to hearing unreal voices, then it is a concern. Otherwise , if it is just you then there's no need for alarm.I would advise you consult a psychiatrist to clarify it.
