Systematic Lupus erythematosus
My niece, 16 years old have been diagnosed with lupus and is under treatment for the last 1 week. DS DNA ++, ANA 1:160 Her symtoms like malera rash, muscular pain and headache accompanied by low temperature is almost reduced to 0 with occasional pain here n there. She is given Omnacortil 40 mg, HCQS 200 MG, micocept S 360 mg along with calcium and antacid and showing reasonable improvment. Under further investigation I t has come that her 24 hour urine test result showing 288 mg which is medium as per lab higher 300 is considered severe. Creatinine is normal at. 6 C 3 and C 4 is lower than range   21.50 and <8 respectively and positive coomb 2 + 1.Question is Has she suffered a kidney lining damage already as diagnosis took time over 2 months. 2. Can it be reversed with medicine? As sge is young, can damaged cell regenerate? 3.how long to get into remission? 4. How to safeguard other dusease as she is taking immunosuppressant. Please help🙏🙏

Hi Had reviewed her history. This is called as juvenile SLE; This is basically an autoimmune disease , where body’s own immunity is acting against self and trying to harm the body more. This could be due to underlying genetic changes and associated triggers like sunlight and other environmental factors. According to your history, there appears to be no major kidney involvement. Proteinuria can be estimated again using 24 hour urinary protein( to collect urine for 24 hours and check protein content). If it is more than 500 mg/ 24 hours then we consider a kidney biopsy and assess the amount of damage. In case there is no proteinuria of > 500 or no albumin in the urine, we usually just monitor. Definitely she has skin involvement, she strictly needs to avoid sunlight which is the most important protective measure to prevent / control flares. Unfortunately, there is no cure but definitely disease can be very well controlled.
Next Steps
Kindly do 24 hours urinary protein. Box is available in any good labs like Lalpath or Anand or metropolis.
Health Tips
Strict sun protection sunscreen is a must daily ( SPF 50+) 3 times a day Use umbrella or long sleeved clothes or hat while in sun. Or better completely avoid sun.
