You didn't mention the age of your grandfather. He is diabetic and hypertensive. He has all the chances of having a stroke. Please don't forget about serious possibilities when you are concentrating on diarrhoea and dehydration. Please consult a General Physician in your area, especially a Cardiologist (if possible) and get the investigations done.

At his age dehydration due to diarrhea and vomiting could result in electrolyte imbalance especially sodium which can cause confusion as one of the symptoms.Consult with his doctor for a clinical examination and blood investigation that include  his serum electrolytes.
