Stage 3B CKD
My father recently received a diagnosis of stage 3B Chronic Kidney Disease (CKD). Notably, his creatinine levels were normal just 1.5 years ago, despite having high blood glucose levels at the time. Although his glucose levels have been well-managed with medication since then, his serum creatinine has risen to 1.9, with a corresponding GFR of 38. Is it possible to stabilize his creatinine levels and halt further progression of the disease?

**Specialist Doctor’s Response:**  **Re: Stage 3B CKD – Stabilizing Creatinine & Halting Progression**  Dear Concerned Family Member,  Your father’s recent diagnosis of **Stage 3B CKD (GFR 38, creatinine 1.9)** despite previously normal renal function is indeed significant. Here’s what you should know:  ### Key Points:  1. **Possible Causes**:     - Rapid progression from **uncontrolled diabetes** (even if now managed) is a leading factor. Other causes (e.g., hypertension, infections, medications) must also be evaluated.  2. **Stabilization Potential**:     - **Yes, progression can often be slowed** with aggressive management of underlying conditions (e.g., strict glycemic control, BP <130/80 mmHg), dietary modifications (low salt/protein), and avoiding nephrotoxic drugs (e.g., NSAIDs).  3. **Urgent Actions Needed**:     - **Nephrology Consultation**: Essential to identify the exact cause, assess for proteinuria, and tailor treatment (e.g., SGLT2 inhibitors/ACE inhibitors if indicated).     - **Lifestyle Interventions**: Fluid intake monitoring, smoking/alcohol cessation, and weight management.  ### Next Steps:  - **Do not delay evaluation**. Early intervention can preserve kidney function and prevent complications (e.g., anemia, bone disease).  - I recommend a **detailed consultation** to review his reports, optimize medications, and create a long-term plan.  For immediate guidance, contact me on WhatsApp at **Eight Zero Zero Nine Eight Two One Three Three Two** to schedule an appointment.
