I weigh 360 lbs, I am insulin resistant but not diagnosed diabetic. I have PCOS and i smoke 7 cigarettes a day. I want to quite smoking and loose weight but can only handle one at a time. Which is more important

Its important to follow healthy lifestyle as fat loss is a gradual process,there is no quick fix approach . Follow a healthy diet which has optimal amounts of first class proteins(dairy,eggs,chicken etc) reduced carbohydrates(white rice,poha etc) & healthy fats(walnuts,almonds etc) along with a structured exercise program that is balanced with weight training and cardio will surely help.
