Severe pain in buttock

I am facing severe pain in my buttock region and my back side of thigh which keeps on shifting in both left and right side.I cant walk,change my position while sleeping.even while sneezing there is adverse pain in my backside. please tell me what disease i am facing and which type of doctor should i consult for my treatment
