Right ovary cyst with 6 week pregnancy.

My wife has 7 week pregnancy with live embryo CRL 7.8 mm. But no sac. Uterus is gravid and right ovary has cyst anechoic cyst of 4.5 cm by 2.5 cm. Which is painful when turn left or right while lying she feel very sharp pain. Also embryo is live with 126 BPM but no sac. Whole stomach especially lower abdomen pains. Yesterday she didn't sleep well due to pain. I m not sure whether to keep or terminate pregnancy in this scenario. I am doubtful whether it is ectopic pregnancy. Ectopic is not detected bcz doctor didn't did TVS scan it's lower abdomen scan. What to do for pain and cyst. And why sac is not seen even live embryo.
