If she is willing to seek help, ask them both to consult a mental health professional to understand what is happening and the remedial measures if needed to be taken.

To know about her mental health and the causes of her actions she has to be evaluated by a professional in detail. Without knowing her reasons and thoughts behind the actions committed by her we cannot comment on what her mental health status is. You or your family would have to bring her and your brother to a psychiatric facility to get them assessed. Why I am suggesting to bring also your brother is because after suffering such trauma any person gets affected and ultimately these results in their dysfunctionality in daily life. And he will also act as a corroborator for the history that your sister in law will give.
