You have mentioned that ear drum is ruptured, I hope the same has been concluded after ear examination by a doctor. Surgery is an option if the perforation does not heal, however, the same surgery will not necessarily help with tinnitus. I recommend you get thoroughly evaluated for tinnitus as the treatment will help if directed towards the cause. Don't worry too much as stress is also known to increase tinnitus.
