"I am currently on the rabies PEP schedule (days 0, 3, 7, 28). I have already taken the first three doses, and my last (Day 28) dose is due in 5 days. Today I became anxious because of my OCD and started worrying about a possible scratch, but when I checked, I found there was actually no real scratch. According to WHO guidelines, in this situation should I just continue with the Day 28 dose as planned, or is there any need to restart or take RIG?"

You are already on the correct rabies PEP schedule (0, 3, 7, 28) and have completed the first 3 doses. If there was no real scratch/exposure, there is no need for Rabies Immunoglobulin (RIG) at this stage. According to WHO guidelines, once PEP is started, it should be completed as scheduled, irrespective of doubts about exposure. Please continue and take your Day 28 dose as planned. No need to restart the course.
Next Steps
• Take the Day 28 dose on time to complete the series.
• No need for RIG since there was no actual wound/exposure and treatment is already ongoing.
• After full course, you will be protected and no further action is needed unless a new definite bite/scratch occurs in the future.
Health Tips
• Do not restart vaccines unnecessarily; it increases anxiety and cost but does not add benefit.
• In case of any real future bite/scratch, wash the wound immediately with soap and water and consult a doctor for advice on booster doses.
• Stay reassured: Completing your Day 28 dose will give you excellent protection.
I can understand that you are anxious because rabies is a serious disease but don't worry as there was no scratch and even if it was a scratch you have been taking anti rabies vaccination according to the schedule so complete the last dose and you are good to go.
