Query about hernia
I have underwent three major abdominal surgeries(midline excision,laparotomy) owing to intestinal problem (the same incision was used for surgery thrice),last surgery date 25thJan 2020. .Since last few months I'm having discomfort in right upper abdomen,just 2cms away from incision..I'm feeling something will bulge through that spot,but there is no visible bulge...it is similar to feeling we get while checking pulse rate...Also while sneezing I get the same feeling...Due to surge in covid cases I'm unable to visit my doctor... Kindly help me...Can it be hernia or it is due to adhesions? Please reply

Yes it might be incisional hernia. Please avoid heavy work, weight lifting and use abdominal binder. You should get an ultrasound abdomen which will support the diagnosis. Remember surgery is the definitive treatment of any type of hernia. But situation is difficult right now, so you can manage your problem with above conservative treatment.

Possibly incisional hernia,use and binder specially in day time,and when comfortable after vivid,visit at surgeon to confirm diagnosis.

From your statement and symptoms it seems to be a self reducible incisional hernia. For now you don’t need to worry , get a abdominal binder and wear it while working , remove it while sleeping. Rest will be sure after physical examination. Get one ultrasound as soon as you can to be sure about it. Can consult me or any other doctor. But Inform your doctor if it hurts or gets stuck there.

yes. it is inciosional hernia   is that hernia is visible on cough and straining? reduced swelling on lying down ? if reduced on lying down - reducible inciosional hernia

Hi... This is Dr.Harsha,consultant Gastrointestinal and Minimal access Surgeon from Bangalore, Laparotomy done through midline incision will predispose to the formation of a midline incision hernia... From your description I suspect that you might be developing an incisional hernia which is protruding through the the fascia at times of increased intra abdominal pressure such as during sneezing causing the above symptoms. I need to ask you a few more questions as well as inspect the wound once before I can suggest whether you require an ultrasound scan or if you can be managed on medications followed by ultrasound scan once  the pandemic stabilizes

Incisional hernia reducible At present looks not an emergency as per ur symptoms Once pandemic settle down then get surgeons consultation and go for surgery either laparoscopy or open as per ur convenience If you get pain and not reducing swelling by its own Then consult surgeon immediately
