Why everyone do not get anal fissure for one time hard stool ??? But I have got a pit as observed by a surgeon in perianal area without getting hard stool after 5 years of Lateral sphincterotomy . Why ??? It has no permanent cure ??

Fissure in ano is not a anal pit. It is a ulcer in anal mucosa that may be acute or chronic.
If you don't have severe Burning pain during anal opening during by stool then chance of fissure is very low.

The pit might be just a normal healing response to the lateral sphincterotomy. It might be caused because of the fibrosis in that region or it might be the area adjacent to the skin tag that is seen in a chronic fissure. The fact that you have undergone a LIS suggests that you had a chronic fissure as it is usually done for chronic fissures and not acute ones where medical treatment is tried first
