Pulmonary Valve Stenosis-Heart murmur
Hi, We have a daughter 9 years old who is having heart murmur since birth. We are following up with pediatric cardiologist regularly for doing 2 D echo since she is having heart murmur. Findings:Mild Valvar pulmonary stenosis-peak gradient 38mm Hg.Pulmonary annulus -15mm,MPA-13 mm We are very concerned about our kids health and need to understand what could have raised this issue and what are the consequences in future. Is there any risk due to this. What precautionary steps we need to follow .

V classify pulmonary stenosis on the basis of gradient, Mild 20-40, moderate 40-60, and severe more than 60. Most of the patient even with severe stenosis remain asymptomatic unless develop heart failure , treatment is indicated when gradient is more than 50 (US guidelines ) in india v follow when more than 60.Treatment of choice is Balloon dilatation. Mild PS as in your child will cause only murmur, less likely it will progress as u said you are following since long. So, it will not cause any harm to your baby, its a innocent disease. Just keep visiting your pediatric cardiologist once a year and rest of the year just forget that your child has some problem.

If it stays mild, you dont need to do anything.. in all probability it will stay mild through out life. When the gradient crosses 80 ,you need to have it corrected. By balloon or by replacement. Balloon valvuloplasty is the preferred approach
