Looks like major cervical incompetance in your case..
I will suggest to start folic acid, ecosprin, vaginal and oral progesterone..
Progesterone Injections are not must coz vaginal has good effect..
Was your APLA test done before pregnancy?
There is also a option of laparoscopic cerclage in pregnancy but as second time it was a twin pregnancy, which is already high risk I m not suggesting that now..
The earlier the cerclage the better.. 14 weeks is good

First avoid stress and anxiety this will help you to prolong your pregnancy,in your case cerclage will help, you can do daily routine activities along with rest after cerclage
Hi. When there is proved case of cervical incompetence then cerclage has to be done . After that rest is necessary with daily activities allowed . Yes you can take progesterone inj weekly once along with oral dose up to 30 weeks . But first do a NT scan and Downs screening .after the cerclage you can start Tab Aspirin 75 till 30 weeks .
