What is the hemocrit I need to maintain. My hemotologist only does plebotomy if my HB is above 16. But many mpn specialists in the USA recommend to do plebotomy to keep the hemocrit below 45. Can I myself go to a blood bank and do venesection to bring the hemocrit to below 45.

No. It shouldn't be done like that. Depending upon races difference, protocol also different between countries. Do phlebotomy under supervision of hematologist only. Your doctor going on currect tract only.
