Plaque is a sticky, soft build up over the teeth, it contains bacteria which can cause numerous dental issues including gum problems and decay. if plaque is not removed regularly by tooth brushing and flossing, it hardens over a period of time to become calculus also called tartar. Now this calculus cannot be removed by brushing so you need a professional help for that. it is very normal for everyone to have plaque and calculus, but to keep it under control you need to brush twice daily and visit your dentist every 6 months for keeping a check on areas which you are nor able to clean by your regular brushing, again its common for most of us.
Before entering a clinic a dentist expects that the patient has brushed their teeth thoroughly, after-all who likes to smell biryani from someone else's mouth(lol).
