I have black colored plaque deposited on my teeth. 2-3 times i have got it cleaned from dentist. But it keeps depositing again and again. Is there any permanent solution?

Plaque is soft that can be removed by brushing...you are probably having stains...
Only way to get rid of stains is by getting a professional cleaning done...
to prevent staining all you can do is avoid food and habits that cause staining...like excess tea, coffee, chocolate, smoking and other forms of tobacco etc...
