Yes, you can take both - Providac (probiotic) in the morning on an empty stomach and metronidazole about two hours after a meal is a perfectly acceptable schedule; just keep a gap of about 2 hours between the probiotic and the antibiotic so the metronidazole does not kill the good bacteria. Diarrhoea running into a second week at 60 should not be treated blindly. The tests that actually help are stool routine with culture and stool for ova/cyst, Clostridioides difficile toxin if you have had antibiotics recently, CBC, CRP, thyroid, celiac screen, and if it still does not settle, a colonoscopy with biopsies - that is the real way to look at the large bowel and terminal ileum. For the small bowel, CT enterography or capsule study is the next step.
Next Steps
Get stool tests and basic blood work done this week, stay well hydrated with ORS, and go to hospital immediately if there is blood in stool, high fever, severe abdominal pain or reduced urine. Book a consult with me on Practo or at Synergy Multispeciality Hospital, Miraj with your reports and I will decide about scopes.
