Looks like the nerve is significantly compressed and if symptoms r severe and it is the first episode with no neurological weakness then there is still chance of conservative treatment . If it is not working then  epidural nerve root block can be considered for pain management after a proper consultation If that also doesn't work then surgery can be considered
