Non healing ulcer on inner foot above ankle with discoloration to skin around the wound and itching. Shooting pain during rest or standing which goes away when walking
A non-healing ulcer above the inner ankle with surrounding skin discoloration and itching is highly characteristic of a venous (varicose) ulcer due to chronic venous insufficiency. The rest pain relieved by walking further supports venous origin, as movement improves venous return. These ulcers need proper wound care, compression therapy, and vascular assessment.
Next Steps
Please see a vascular surgeon urgently for Doppler ultrasound of the leg veins and proper ulcer management â non-healing ulcers worsen quickly without treatment. Please consult me directly on Practo for a detailed evaluation.

The ulcer location (medial aspect above the ankle), surrounding skin discoloration, itching, and chronic non-healing nature are highly suggestive of a venous (stasis) ulcer with chronic venous insufficiency. However, the complaint of shooting pain at rest or standing that improves with walking is atypical for a venous ulcer and warrants evaluation for neuropathic pain, lumbar radiculopathy, or less likely peripheral arterial disease. A thorough vascular examination (including palpation of peripheral pulses and ABPI) is essential before initiating compression therapy.
Next Steps
Consult a General Surgeon/Vascular Surgeon.
Clinical examination of lower limb veins and arterial pulses.
Venous duplex Doppler to assess venous reflux.
Screen for diabetes (HbA1c), CBC, renal function, and wound swab only if there are signs of infection.
Appropriate wound care , initiate graduated compression therapy, leg elevation, calf muscle exercises, and treatment of venous insufficiency.
Health Tips
Do not start compression bandaging until arterial insufficiency has been excluded.
Seek urgent medical attention if the ulcer rapidly enlarges, develops foul-smelling discharge, fever, increasing redness, severe pain, or black discoloration of the foot.
Persistent ulcers (>3 months) or atypical ulcers should be biopsied to exclude malignant transformation or other uncommon causes.
