My sister fell at home and had swelling. The xray reports are attached. The CT scan impression says Non displaced fracture involving talus and fracture involving 2, 3 and 4th metatarsal bones.
What does that mean. Is surgery mandatory. Please advice.

If physical demand is not much and if you are not concerned with a slight bump on the foot...then you can be treated with out surgery by means of a plaster cast.

The fracture of 3rd and 4th metatarsal base appears to be slightly displaced.
Ideally it should be reduced and held in place with K wires.
If it is not operated, there is a possibility of malunion and it may cause some visible deformity in some cases.
