I don't know who you are. I have never seen you. Frankly I don't even know whether you have two legs or one leg.
So when some rules have been framed against giving Medical Certificate by Tele-consultation there is a reason behind it.
Please follow the laws of the land and consult with a General Physician in your area who can examine you physically.
