Surgery is absolutely required as there a bit of subluxation and the fracture is displaced which needs to fixed by a CC screw. Breast cancer on the same side is not a contraindication for surgery. But need to get a CT brain I/V/O Brain Haemorrhage before proceeding with procedure.
Next Steps
Can consultant an orthopaedic surgeon and plan accordingly

The fracture is displaced quite a bit with subluxation of the joint, which means fracture healing without surgery will be questionable.
Next Steps
Requires surgical fixation, provided she's fit for surgery. She will have better quality of life after surgery as the chances of fracture healing properly are higher. Trust your treating orthopedic surgeon and the anesthesia team to take the best call and follow their advice.

Surgery is a must for her. It doesn't really matter which side she's having the breast cancer. The only thing you need to worry about is her fitness for surgery. Physicians and the anaestheologist will give their opinion prior to surgery. For her to have a good performing shoulder, surgery is a must
Next Steps
she requires immediate surgical intervention. You will have to take the risk and get her operated as early as possible.
