I could see a mole on my head and it pains whenever I touch. There is a catch, if I wash my head it won't pain at all. Sometimes it doesn't even pain for days. This pains only when I touch it. Is it something serious? Or what it is?

Based on the image, I do not see any obvious features suggestive of a serious skin lesion, which is reassuring. The pain is more likely due to local irritation or inflammation of the mole or the surrounding scalp rather than the mole itself.
Try to avoid repeatedly touching or scratching the area, as this can aggravate the pain. If the mole increases in size, changes in color or shape, starts bleeding, or the pain persists or worsens, consult a dermatologist for a clinical examination.
Proper evaluation and personalised advice are needed for your concern. A detailed consultation would be best to provide accurate guidance.

Hello. A painful mole or nodule on the scalp that experiences varying sensitivity—especially feeling better after washing your head—is frequently linked to localized irritation. This can occur due to mild hair follicle inflammation (folliculitis) right beneath the mole, a small traumatized sebaceous cyst, or an irritated intradermal nevus (raised mole) secondary to combing or friction. While it is highly likely to be a benign inflammatory response, any mole that exhibits persistent pain upon touch should be properly evaluated.
Next Steps
Recommended Actions:
Avoid Irritation: Do not scratch, pick, or squeeze the mole, and be very gentle while brushing or combing your hair over that specific area.
Track the Changes: Observe the mole closely for any alarming signs, such as rapid growth in size, irregular or blurred borders, changes in color, bleeding, or fluid discharge.
Clinical Evaluation: Schedule an in-person physical checkup with a dermatologist to get a dermoscopy analysis to ensure the skin architecture is entirely healthy.
Health Tips
Fluctuating tenderness often points toward simple inflammation rather than deep-seated issues, but an accurate visual assessment by a professional is essential for complete peace of mind.
