There are n number of causes. Might be Cervical PID or Should Conditions. Important is your posture. Check whether your scapula bone(triangular bone) is in symmetric from Central. Whether your involved side shoulder is elevated or protracted.

But this is radiating pain so might this could b related to arround C3C4 cervical nerve iritation or disc or inflamation..
