Dear mam, at 41 years, as per your words it's intense pain, radiating towards sides, since 3 years itself says go for MRI- to find out the source of pain, and according to clinical and MRI correlation will be able to find out the pain generators and will be able to manage it.
Till then continue medicine which was prescribed by your consultant and do follow rehab program.
