Coronal images of the hip and sacro-iliac joints reveals: Abnormal marrow signal intensity involving posterior and superior left acetabulum. Osteolysis with cortical erosion is also noted with fairly well-defined abnormal soft tissue incontinuity in the gluteal region, predominantly in gluteus minimus muscle, suggestive of abscess / granulation tissue. It abuts the pyriformis muscle. Medially it extends upto the sciatic foramen abutting the sciatic nerve. Findings most likely suggestive of infective etiology like Koch’s. Suggest: clinico laboratory correlation.
