I did a MRI scan report attached and want to know what exactly mentioned. Is the report shows fistula. Whether I need surgery.please advice.
MRI Reports Indication -
images reveal large curvilinear sinus tract arising from subcutaneous plane of right gluteal region extending superiority and then anteriorly upto the level of leavtor ani muscle with translevator extension and secondary ramifications as described - suggestive of grade - V impending fistula.

Your mri report description s/o complex high lying anal fistula .
Fistula do require surgery, no medical cure. Only to control symptoms medicine is used.

Yes , this fistula tract has to be surgically removed either By a Single stage or 2 stage surgery , there are non surgical procedures also available but with high recurrence rate.
