Medical certificate for leave
Looking for a psychiatrist for stress, anxiety and depression, along with regular therapy sessions. I work for a US-based company and am currently on approved medical leave until September 15. I may need to extend my leave for another 2 months based on my doctor's assessment. I am already taking medication and would like to continue treatment with a psychiatrist. I was also recently diagnosed with severe obstructive sleep apnea (AHI 63.6, oxygen desaturation 77%). My employer requires medical documentation such as consultation notes, clinical assessment, treatment plan and, if medically necessary after evaluation, a medical certificate supporting the leave extension. ** recommend a psychiatrist who can provide ongoing treatment and appropriate medical documentation based on clinical evaluation? I am comfortable with either in-person or video consultations.**
