Ur ultrasound report normal.
It's anomalies scan that show no gross anomalies.
Regarding ur low lying placenta u should do rescan @28 week to rule out placenta previa.

A placenta which is 4.5 CM away from the internal OS is not at all lowlying..so this ultrasound is absolutely normal and there is nothing which you should be worried about...
kindly ask your ultrasonologist to reconfirm if she really meant 4.5 CM of 4.5 mm
