Pain initially after extraction might be due to the angulation ( tilted) of the wisdom teeth because of which being a difficult extraction the pain sometimes might get referred to your adjacent teeth and ear. This usually subsides after few days depending upon the severity with proper antibiotics course. Regarding the recurrence of pain , as per the OPG enclosed I think your second last teeth will be requiring root canal treatment ( second molar teeth). Still to be doubly sure please get a repeat OPG done. Also it’s better to have a ENT clearance also.
Next Steps
OPG repeat. Also do warm saline rinses 3-4 time daily

The pain may be due to the decay in lower left second molar tooth, which need to be addressed. For now you may take pain killers like dolo 650  three times a day after food Or ketorol dt twice daily after food. Avoid biting from that tooth Maintain good oral hygiene Do warm salt water rinse

I think so this opg is before extraction, can I see post extraction opg, since you said there is a lymph node swelling which is a bad sign. Try getting a fresh opg done
