Better to go for surgery ie total knee replacement . In this stage of osteoarthritis disease , injection wont give full relief of pain as the osteoarthritis is in advanced stages. If his general condition and medical problems are not permitting for surgery then he can be offered intrarticular injection of steroid or prp injection .
Next Steps
medical checkup for fitness of surgery

Can use varus knee brace and continue physiotherapy exercises and lifestyle modification, and if still not relieving then have to undergo surgery with risk .
