Knee injury, meniscus tear
Good afternoon, I am attaching this report. Please suggest me appropriate treatment except TKR. H/o fall 2 times in March 23, age 60 yrs female, since then pain and swelling in knee and thigh.  restricted walk only her daily work . Took all the medicines , fomentation but nothing works as expected. Please guide me with your expert advice. IMPRESSION: MR Imaging reveals joint effusion with: *O.A changes tibio-femoral compartment with grade IV chondromalacia patellae. *Degenerative horizontal tear posterior horn and body of medial meniscus. •Early myxoid degeneration in lateral meniscus. Myxoid degeneration in ACL and PCL. *Degenerative bone marrow edema in tibiofemoral bony margins. TIA.

At 60 years, if there are grade 4 OA changes and if you have additional meniscus and ligament tears, the best treatment is TKR. There are numerous patients getting operated for TKR and they have excellent results. However if you don't want TKR now, only the ligament injuries can be taken care of by key hole surgery. But at 60 years the outcome may not be as good because of weak bones and relative inactive lifestyle. If not now, then in another 5 years she will require TKR
