Once a creatinine has been elevated for 3 or more months, it is termed Chronic kidney disease. It is unlikely that it returns back to normal, he needs medication for preventing it progression. Diet modification to avoid high protein and hypertension control with few medications. Consult a nephrologist for further plan and preparation.
