Sir, mri report of whole abdomen is attached. My question is that whether any operation is required or not. Also, is there any possibility of recovery of kidney by insulin?

The features in MRI is suggestive of chronic kidney disease. You better be in follow up with nephrologist. No urological intervention would help him. Better to have a good glycemic control and control of blood pressure if he is a hypertensive.
