Most likely you are having Anal fissure. You should apply some soothing ointment or coconut oil in anal region after taking sitz bath twice daily. For easy bowel movement, you may take some stool softening medicine like Haritaki Powder. For further support you may call reception at Sushruta Ano Rectal Institute.

Itching in back may be fissure in ano Keep a check on your diet . Eat healthy food with small meals at small intervals . Avoid all the maida , oily , fried and spicy food . In Ayurveda we have very good treatment for all these problems.

Clean the area properly and flash warm salt water after wash. Take triphala choorna for 7 days if the evacuation is incomplete.
Dose 1tsp with warm water just before sleep.

Take Laghusutashekar ras one tab twice a day before food..
Amlapittantakras one tab after food twice a day.
Drink plenty of water 2-3 ltrs .
Drink buttermilk 200 ml / day.
Eat plenty of vegetables,fruits and greengram sprouts everyday
Walk,yoga,exercise for an hour everyday
Sleep leisurely for six hrs atleast at night.
Avoid spicy oily foods,preserved canned foods,bakeryfoods,fastfoods.
Include two tsf ghee everyday food.
