Looks like severe B12 deficiency, do cbc and b12 levels. Eat soft food, avoid junk and plenty of water. But get properly checked by your nearest physician first

Dnt worry. Tongue examination reveals a lot. Check for tongue ulcers. Give child lots of liquids to drink. Also Vit B 12 supplements. Avoid junk food and include bananas in the diet.
