It is usually common that symptoms of OCD may increase under stressful conditions like exams. Your brother needs to consult Clinical Psychologist for CBT sessions along with medications. Combination therapy is highly effective in OCD management.
Next Steps
consult Clinical Psychologist also with medication

I feel , the stress of preparing for the NEET  itself would have caused him OCD. The Internal fear of passing through the exam, is to be handled. I am sure all your family members  and friends would have helped him. Unless he willing to accept himself regarding whatever  the result could be, it is difficult for him to handle this stress.
Next Steps
Do fix an appointment with a Psychologist and help him to share his feelings . That might help him to cool his thought process and make him more relaxed.

It sounds like a worsening scenario and can hinder stand the distress faced by cares and family. Please understand that this forum is not adequate to discuss and suggest treatments for what is a severe illness.
Next Steps
Do go back to the treating psychiatrist for what can be done next? If you are not happy with response chose to have a second opinion.
Health Tips
Stop at second opinion and stick to treatments both medication and therapy. It will take time.
