Instances of sudden pulse spike
For the last 2 months there have been instances where my pulse has suddenly spiked causing vertigo uneasiness and the feeling of something bad is going to happen and it lasts till atleast 30-45mins then I'm back to normal...These instances are especially after caffeine and smoking although I used to consume both earlier also but there were no such spikes...My resting pulse is around 80 and bp is 115/75 normally and my ecg is also alright...Then why these spikes?

Caffeine, tobacco and anxiety are the likely triggering factors. Get ecg and cardiac echo done and share results.

Most likely the spikes are due to some underlying anxiety. Is there any recent changes in mood or sleep present along with this. Is there anything that happened before this started maybe something that was stressful or something that is ongoing something you are scared about. The thought patterns you are having are called as anxious premonitions in which we start thinking all the possible outcomes to decrease uncertainty and somehow get stuck on the worst outcomes. This is accompanied by various physical symptoms which vary in people like increased heart rate, breathlessness, sometimes trembling of hands or maybe dizziness like in your case. Sometimes these symptoms can be due to some underlying pathology like arrhythmias which needs to be assessed. Anxiety is completely treatable if addressed properly in the initial phase. With some behavioural and thought pattern recognition you can even manage it without medications. Consult for proper evaluation and management.

The sudden pulse spikes, vertigo, and 'feeling of impending doom' (uneasiness) you described are hallmark symptoms of Panic Attacks or Paroxysmal Tachycardia. Since your resting ECG and BP are normal, your heart is likely healthy, but it is overreacting to stimulants. Caffeine and Nicotine (from smoking) are powerful vasoconstrictors and cardiac stimulants. Over time, your body's 'Tolerance' can change, meaning what was fine earlier can now trigger an exaggerated 'Fight or Flight' response, leading to these 30-45 minute episodes of intense anxiety and rapid heart rate."
Next Steps
Square Breathing: When you feel the 'uneasiness' starting, use the 4-4-4-4 technique (inhale for 4s, hold for 4s, exhale for 4s, hold for 4s). This physically signals your nervous system to calm down. Identify the Pattern: Keep a diary of exactly when these spikes happen. Check if they are linked to lack of sleep, dehydration, or specific stressful thoughts. Stimulant Detox: Strictly avoid caffeine and smoking for the next 2 weeks to see if the frequency of these episodes drops. This is the most effective diagnostic tool you have right now.

Please go for a proper consultation for further evaluation. A detailed history and examination are needed to understand your symptoms correctly. Without this, it’s difficult to give the right treatment. Kindly bring any previous reports or prescriptions.

Because of smoking and caffeine .they are enzyme inducers and stimulate the functions .so heart function is increased and that's why you see this fluctuations in your beat.in earlier body manage but now you are chronic smoker that's why body not tolerate easily ,so better if you quit or stop it.
