Infertility
We have been doing follicular study for two months. The first cycle was unsuccessful. In the second cycle, my wife's SSG was normal and both tubes are open. AMH is 2.87, thyroid is controlled at 2.38, and she is taking Trafolic, Ecosprin and Fertisure Fem. She got a trigger injection on Day 13, ET was 9 mm, and corpus luteum was seen on Day 16. Today is Day 29, 15 days after the trigger, but UPT is negative. My semen analysis shows 16 million count and 2% rapid progressive motility. She has lower abdominal pain, breast tenderness and white discharge. Everything seems normal, so why is pregnancy not happening? She is very depressed after seeing the negative result. What should we do next?

Two monitored cycles is a short run, and one unsuccessful cycle tells you very little. With normal tubes on HSG, the usual next questions are whether ovulation is genuinely occurring and releasing well on the scans, whether the semen parameters have been checked recently, and whether the lining is responding. Most couples who conceive with ovulation induction do so across three to four properly timed cycles rather than the first one, so the plan is generally continued rather than changed after a single miss.
Next Steps
Ask your treating doctor to confirm a recent semen analysis is on file, and to review the ovulation and lining findings from both cycles before deciding whether to continue the same protocol, add insemination, or change approach.
Health Tips
If three to four well monitored cycles pass without conception, that is the point to discuss the next step rather than repeating the same cycle indefinitely.
