I get pain when I press on my chest

Hi Doctor, I get pain when I press on my chest where heart is and when I sleep on bed reverse on my chest..I get sever pain. This is there from long time. I need your help to know what it is ..please refer me the right thing to do now sir. Thanks in advance.
