I am having continuous hair fall.

I have been suffering from a very bad hair loss problem for last 3-4 years. Earlier it was not that serious but in last few weeks it has gone severe. I am afraid within a few months i will be completely bald, I have a constant problem of dandruff that is troubling me continuously. Please help me out.
